Waiver
Adults age 18 years and older must complete, sign, and submit their own waiver. Minors under the age of 18 must have a waiver completed and signed on their behalf by their own parent or court-appointed legal guardian. Waivers stay active in our system for three (3) years before needing to be renewed. Climber's Rock reserves the right to request a government issued photo ID to be shown to verify age.
INSTRUCTIONS FOR PARENTS filling out waiver(s) on behalf of your child(ren). Enter YOUR NAME and YOUR DATE OF BIRTH FIRST. Your CHILD'S (or CHILDREN'S) name(s) and birthday(s) will be entered after.
IMPORTANT!
1) Waivers for minors must be completed, signed and submitted by their own PARENT or COURT APPOINTED LEGAL GUARDIAN.
2) Waivers for minors signed by anyone else (e.g. friend, relative or group leader) WILL NOT BE ACCEPTED.
